Purpose of this Authorization

This authorization allows Asian Egg Bank and its affiliates (“AEB”) to use or disclose certain information, images, or content provided by me for marketing, promotional, or educational purposes.

The intent of this authorization is to allow Asian Egg Bank to share authentic stories, donor experiences, intended parent experiences, family milestones, and educational content in alignment with its mission to support individuals and families throughout the donor egg journey.

Information to Be Used or Disclosed

I authorize Asian Egg Bank to use the following materials that I voluntarily provide or that are created in connection with my participation, experience, or relationship with Asian Egg Bank:

• Photographs, video recordings, or audio recordings of me.
• Written or recorded testimonials, quotes, reviews, or statements I provide.
• Images or content voluntarily submitted by me, including baby photos, birth announcements, milestone images, donor-related content, family photos, or other submitted materials.
• Educational or promotional content created with my participation.

No medical records or identifying health information will be shared unless I have specifically and separately authorized it. Any materials used will be limited to those relevant to promotional, educational, informational, or awareness-related purposes.

Authorized Uses and Disclosures

I grant Asian Egg Bank and its affiliates a non-exclusive, perpetual, royalty-free license to use, reproduce, publish, edit, display, and distribute my testimonial, image, likeness, voice, story, and submitted content in any media format, including but not limited to:

• The Asian Egg Bank website and affiliated web pages
• Social media channels, including Instagram, Facebook, YouTube, LinkedIn, TikTok, and other platforms
• Printed brochures, newsletters, flyers, and educational materials
• Digital advertising, email campaigns, presentations, and promotional campaigns
• Internal communications, training materials, and professional education
• Event materials, booth displays, and clinic or partner-facing materials

All use will be consistent with Asian Egg Bank’s standards of professionalism, privacy, dignity, and respect.

Waiver and Release of Claims

I waive any right to inspect, approve, or claim ownership of the materials produced or published using the content I provide.

I release and discharge Asian Egg Bank, its employees, representatives, contractors, agents, affiliates, partner organizations, and related entities from all claims, demands, or causes of action arising out of or related to the authorized use of my image, likeness, voice, testimonial, story, or submitted content. This includes, but is not limited to, claims of defamation, invasion of privacy, infringement of publicity rights, or violation of any rights related to the use of the authorized materials.

Voluntary Consent and Revocation Rights

I understand that this authorization is voluntary.

My decision to sign or not sign this form will not affect my ability to work with Asian Egg Bank, receive services, access donor information, participate as a donor, or pursue donor egg services, unless the use of my image or testimonial is specifically required for a separate program or agreement.
